Starting November 12, 2018 Urology San Antonio is implementing new Electronic Medical Records software. We have partnered with iSalus Healthcare™ to create a customized, Urology-specific system called UroChoice™ that will help us better serve our patients.
The new system allows patients and providers more organized and efficient access to medical records, improved communication between healthcare teams and patients, as well as an enhanced patient portal, called MyMedicalLocker™ that will be easily accessible via computer or mobile app.

The previous Patient Portal will still be available for a limited time for historical purposes, however you must use MyMedicalLocker to create new appointments, keep track of vital information, and communicate with the practice.
The patient portal allows you securely access your health information and better manage your care. Having direct access to your health records (including labs and clinical summaries) and the ability to update contact details, allergies, medications, and other important health information will allow you to take greater control of your care an increase the the accuracy and communication between you and your doctor.
Although most information may be added through the patient portal, we still will need a copy of your insurance card and driver’s license, your signature on the patient consent for treatment form, and for you to complete a brief recent symptom form when you arrive.
